U.R.I.S. is a provincial program that supports children with specific health care needs when they are attending community programs such as school or childcare facility. Through URIS support, a Registered Nurse develops a health care plan for the child and trains staff on how to care for the child while he/she is at school/childcare facility. The school/childcare facility receives U.R.I.S. support from the Interlake-Eastern Regional Health Authority (IERHA).
There are many documents that are currently in use. The Group B application form is updated annually and is for specific health care concerns as outlined in the attached application form. The other document is a sample of a Student Specific Individual Health Care Plan. Every IHCP has the same first demographic-contact page followed by the specific diagnosis information.
